چکیده

To determine the coagulation defects associated with massive blood transfusions, coagulation studies were performed on 21 battle casualties admitted to the US Naval Support Activity Hospital, Da Nang, Vietnam. All but one patient who received less than 20 units of Acid-Citrate-Dextrose blood (7 patients) did not develop a coagulopathy. All patients who received more than 20 units (14 patients) developed a clinically significant coagulation defect. Although the partial thromboplastin and prothrombin times were markedly prolonged (i.e., low Factor V and XIII levels), restoring these times to normal levels by fresh frozen plasma administration did not terminate the clinical coagulopathy. In all 12 patients who had platelet counts less than 60,000/mm(3), a clinical bleeding problem (coagulopathy) developed. The coagulopathy eventually spontaneously resolved (n = 4), was successfully treated with fresh blood (n = 4), or the patients died (n = 4). A mathematical analysis confirmed that the thrombocytopenia is dilutional in origin and is the primary cause of a coagulopathy from massive blood transfusions. The authors conclude that clinically important coagulopathies predictably occur after administration of 20-25 units of stored Acid-Citrate-Dextrose blood in acutely wounded, previously healthy soldiers. Fresh frozen plasma should not be a major therapeutic choice for coagulopathies in massive blood transfusions. Treatment of dilutional thrombocytopenia (50,000/mm(3)) is a primary component of treating coagulopathies associated with massive blood transfusions.

Fresh whole blood transfusion: a controversial military practice.

The transfusion of fresh whole blood (FWB) for trauma-induced coagulopathy is unusual in civilian practice. However, US military physicians have used FWB in every combat operation since the practice was introduced in World War I and continue to do so during current military operations. We discuss our review of all blood products administered to US military casualties in Operation Iraqi Freedom ...

Transfusion: -80°C Frozen Blood Products Are Safe and Effective in Military Casualty Care

INTRODUCTION The Netherlands Armed Forces use -80°C frozen red blood cells (RBCs), plasma and platelets combined with regular liquid stored RBCs, for the treatment of (military) casualties in Medical Treatment Facilities abroad. Our objective was to assess and compare the use of -80°C frozen blood products in combination with the different transfusion protocols and their effect on the outcome o...

Damage control resuscitation: a sensible approach to the exsanguinating surgical patient.

BACKGROUND The current wars in Iraq and Afghanistan have resulted in the highest rates of combat casualties experienced by the U.S. military since the Vietnam conflict. These casualties suffer wounds that have no common civilian equivalent and more frequently require massive transfusion (greater than 10 units of packed red blood cells [PRBCs] in less than 24 hrs) than civilian injured. DISCUS...

Meta-analysis of plasma to red blood cell ratios and mortality in massive blood transfusions for trauma.

BACKGROUND The current military paradigm for blood transfusion in major trauma favours high plasma:RBC ratios. This study aimed determine whether high plasma:red blood cell (RBC) ratios during massive transfusion for trauma decrease mortality, using meta-analysis of contemporaneous groups matched for injury severity score. METHODS A systemic review of the published literature for massive bloo...
